Hello everyone, I’ve recently been dealing with intense pain in my lower abdomen and testicles. A neurography revealed that while the lumbar plexus branches look fine, there is thickening and hyperintensity in both genitofemoral nerves starting from the psoas muscle, including the genital branch after the inguinal ring. It’s a bilateral, asymmetric finding. The diagnosis is neuropathy of the genitofemoral nerves on both sides. I’ve tried pregabalin, duloxetine, and tramadol without success, and tapentadol barely helped. Currently, I’m using 20mg buprenorphine patches, 1g of paracetamol every 8 hours, and 50mg of tapentadol for breakthrough pain. Nerve blocks in the spermatic cord were successful, reducing the pain to zero. I work as a guide walking 20km a day, but I can’t do that now. I’m at my wit’s end. I’ve requested a triple neurotomy or even a radical bilateral orchiectomy—I’d happily live on synthetic testosterone if it meant relief—but the doctors refuse, even though long-term opioid use isn’t a viable solution. I’m worried a neurostimulator might dislodge because I’m always on the move. I feel very hopeless and don’t know how to keep going.

I am so sorry you are going through this. My nerve pain shows up in strange places—I actually had my tongue go numb during a work meeting once—but I can’t imagine the pain you’re describing. Is medical THC available where you live? I’m in California, and I use gabapentin during the day and a THC tincture at night so I can sleep well enough to function. I really hope you find some relief soon.
Hi, what kind of medical imaging or test did you have to check the genitofemoral nerve?
