Has anyone undergone surgery despite having a normal EMG? (My ultrasound shows segmental enlargement and my MRI shows nerve signal hyperintensity)

Hello, like the title says, I’m experiencing ulnar nerve symptoms in both arms, along with median nerve issues and tendinitis, yet my EMG results have been normal for six years. That said, several ultrasounds have indicated segmental enlargement of the ulnar nerve near the cubital tunnel/medial epicondyle, and MRIs have shown signal hyperintensity in that same spot. I’m also aware that I have ulnar nerve subluxation, which is more severe on my left side. Has anyone else had a successful transposition or surgery even with a negative EMG? If you did, were your ultrasound or MRI results positive? What was the outcome for you?

My EMG came back normal, but I pushed for further testing because I was certain something was off. After I described my symptoms to the radiologist, the ultrasound revealed subluxation. I didn’t just stay quiet during the scan; I explained the situation and asked for proof because I knew I wasn’t imagining things. I think I had a skilled radiologist who understood where to check and how to identify the problem. I had a submuscular transposition this past May and am currently in recovery. I’m happy with the decision to have surgery. The numbness disappeared shortly after the procedure, and now I’m just working through some muscle stiffness.

Yes, I had it on both sides, though I only ever had an EMG for my right arm. I didn’t even bother testing the left one and proceeded straight to surgery following the MRI. I didn’t have any ultrasounds, only MRIs.

Sort of! Here is what I have mentioned previously: I have had several negative EMGs despite clear symptoms, with only one ever showing a mild positive. My first release surgery was performed based solely on a physical exam, without an EMG. I also underwent carpal tunnel release despite negative EMG results because the symptoms were obvious. I had another cubital release in 2023 following that one mildly positive result (which was significant for me, considering my history of negatives). My most recent EMG was negative again, but the symptoms persist, and an ultrasound confirmed nerve instability in my right arm.

That was me! My EMGs were normal, but the ultrasound showed a very inflamed ulnar nerve. I had a transposition last December and I’m feeling much better now.
