Deciding between daily antivirals and episodic treatment is a personal choice that depends on your frequency of outbreaks. Here is a quick guide to help you choose.
First, track your outbreaks for six months to establish a baseline. If you have more than six a year, daily suppressive therapy often significantly reduces the viral load and transmission risk.
Second, talk to your doctor about kidney function before starting long-term meds. Finally, weigh the cost and daily commitment against the peace of mind. Both methods are valid, but consistency is key for whichever path you choose.

Suppressive therapy was a total game changer for me. I went from having a sore every two months to zero in the last year.

I personally prefer episodic. I only get one or two outbreaks a year, so I don’t see the point in taking a pill every single morning.

Do people worry about long-term side effects? My doctor said it’s fine, but I’m still a bit hesitant about being on medication for years.

The transmission risk reduction is the biggest factor for me. Even if I don’t have symptoms, knowing I’m less likely to pass it to my partner is worth the daily pill.

Does anyone feel like it loses its effectiveness over time? I’ve been on daily Valtrex for 3 years and lately I feel like I’m getting prodrome symptoms more often.

Helpful post! I just started episodic treatment last week. Fingers crossed.

I tried episodic for a while but I was always too late to take the pills. You really have to catch it at the very first tingle or it doesn’t do much. Daily is much easier for my lifestyle because I don’t have to carry a ‘rescue pack’ everywhere I go.

Cost is definitely a thing. If you don’t have good insurance, those daily pills add up. Episodic is way cheaper if you’re rarely symptomatic.

I disagree with the 6-outbreak rule. I only get 3 or 4, but they are extremely painful and last two weeks. Suppression was the only way to get my life back.

Is there a big difference between Acyclovir and Valtrex for daily use? My pharmacy keeps switching them.

I just hate the anxiety of wondering if today is an outbreak day. Daily meds take that mental load away for me.

Great advice on checking kidney function. It’s rare to have issues, but staying hydrated and getting bloodwork once a year is just smart practice.
