Nerve pain intensifying despite improving glucose levels

I was diagnosed with type 2 diabetes and neuropathy back in August. Initially, my A1C was near 12, and the nerve issues were just minor tingling and numbness. However, for the past three weeks, the pain has turned excruciating. It feels like my toes are being wrenched off with burning pliers while fiberglass is being rubbed into my ankles and the tops of my feet. My recent labs showed my A1C has fallen to 5.2. I’ve been dedicated to managing my condition through exercise and have dropped roughly 40 pounds. My physician switched my prescription from gabapentin to Lyrica, but the agony is so intense I can’t get any sleep. Does anyone have tips on managing this or an explanation for why the pain is spiking even though my numbers are improving?

When nerves are dying, you lose sensation, but the healing process and restoration of function can be incredibly painful. It usually takes quite a while before the discomfort subsides.

What medications are you on? Are you using Metformin? Also, are you supplementing with vitamin B12?

The only way I can get some rest is by taking 300mg of gabapentin—some days, it literally feels like I’m stepping on sharp stones.
